Any hearing aid benefit that you have under your insurance plan will be discussed during your hearing aid consultation appointment.
We need the following information to check on your hearing aid coverage:
- Primary and secondary coverage details
- Member ID number and Group ID number
- Provider services phone number on the back of your insurance card
